In big bands? Usually because record labels don't like it. For example, look at how Virgin refused to put any financial muscle behind promoting Gary Moore in the US during the '80s because they felt he was too unattractive to crack MTV. Bands themselves are often quite wary of it themselves, too. A then-unknown David Coverdale very nearly didn't the Deep Purple job because Blackmore thought he was too fat and ugly to front the band.

But outside of major label groups, there's plenty of ugmos around. Just look at the punk scene.
