The cancer thing in Hospice is just a metaphor for a toxic relationship. It's actually pretty blatant if you pay attention. It's still a sad album, but it's not near something like A Crow Looked At Me which is literally about the aftermath of the love of someones life slowly dying from cancer in front of them.

Stage 4 is beyond the extrinsic reasons of being sad. It's the intrinsic loathing from 3 married with the random extrinsic pain and suffering from 2 that moves the sadness from one of challenge and understanding to one of complete and total hopelessness. Stage 4 is giving up on anything and everything- yourself, the world, people, hope, happiness, literally everything.

I think the chart is flawed. "I quit" is two subjective, and light. What you just described is at least stage five, past suicidal thoughts, right into nihilistic existence for the soul purpose of inking out and existence, devoid of thought, feeling, and reason.
Many people have quit life after the death of a loved one, but not many normal people are ever pushed to a level you describe.
I think it is stage four, by stage four's description.
But it is not the lowest you can go.

stage 1 description sucks because it passes off an album as overemotional
i guess my stage one albums for me are either deceptively sad in either a lyrical or musical context, or they reflect on humanity as a sad concept
stage one should be like that because albums are never objectively pandering to an audience seeking an emotional album

That's what I thought the first time I listened to Confield but trust me when I say it's one of the most brilliant albums I've ever heard. The atmosphere and sound design on this album are so far beyond anything else ever made.

This is my favorite and probably one of the most accessible on the album. Starts out with some ominous space-y synths and eventually evolves into something that sounds almost like screams of pain.

Anytime, I'm glad I could show you something new. Honestly I thought I had been duped when I listened to Confield for the first time but now it's one of my favorite albums. I've listened to all of Autechre's discography and plenty of similar artists in the genre and I have yet to find another album like this one.
