He knew he had cancer but it wasn't terminal then.
He was diagnosed in mid 2014, by mid 2015 he was in remission, and it came back and spread everywhere in November 2015. He recorded Blackstar in early 2015, when he was fighting cancer because it wasn't terminal yet.
I thought this was common knowledge. Almost everywhere I've read says it became terminal around November.
