Johnny Cash's is simplified for the masses which leaves out the meaning of the original for something much simpler. Original is much more unique and has much more of a punch, Johnny Cash's vocals aren't that great in Hurt, without music video this cover is not all that powerful, it's all about that goddamn amazing music video which people refuse to give proper credit for, if that music video wasn't there, this would not be as effective.

The entire Downward Spiral album is literally a downward spiral into suicide. Hurt is the closing note of regret. No doubt it was sad Cash was sick and dying when he covered the song. The wish to escape from being controlled isn't tied to just being a teenager, or young. The Dowwnard Spiral's idea is that every one controls you and always will be controlled, every one in the world wants your personality to be suited to their own likings, your lover, friends, religion, goverment. This isn't exclusive to teenagers, it is very simple and does not require any thought to write off anything as "edgy teen" stuff.
Writting of Trent's vocals as "crange inducing melodramatic" is the same as writting of Cash's vocals as monotoney and lifeless, both of them serve their purpose. Both meanings of the songs are rich and are very personal. I do not know how you can say "emotional depth of a teacup" when you consider that NIN's Hurt is an ending of The Downward Spiral where the protagonist looks back at all what has happeend, connecting everything flawlessly together and closing the album.
