Some hip-hop is make believe, essentially fiction. Other hip-hop is very sincere and comes entirely from personal experience. This has always been the major divide in the hip-hop community, between real rap (they actually mean what they're saying) and poseurs (just make believe). Over time the divide between the two has gotten fuzzier, hip-hop artists eventually stopped caring as much about how they were perceived or got more abstract about their lyrics. The focus in hip-hop today leans more towards storytelling.

Pretty cool of you to mention Skepta's album OP. I went back to it about two weeks ago having not listened to it for at least 3-4 months and it still holds up quite well.
