People keep repeating this as if it really matters, but anyone with a little bit of medical knowledge or firsthand drug experience is aware of this thing called tolerance. At this point, I'm sure his addiction had progressed to the point where normal standard values didn't apply -- "3x the lethal dose" was probably just a heavy amount for him but likely not enough to necessarily kill him.
