It really seems like Brian is in a great place right now and the notions about a severely deteriorated mental state mostly stems from observations made about him when he's on stage/interviewed, neither of which he was comfortable with even at his peak. I'm not sure about his autobiography because of that possibility of ghost writing, but the exerts I've read point towards the same conclusion.

Nonetheless the man is 74 and he's doing incredibly well all things considered.
