Celestia makes such a great fatty. I think part of it is the realization that that level of gluttony pretty much necessitates her giving up her active role in running Equestria. She's a pretty devoted monarch in the show, but you can imagine her throwing it all away from an overpowering urge to stuff her face. That devotion to gluttony is pretty hot.

It would be pretty neat to see a story where Celestia's turned the country over to Luna, and Luna's a serene absolute monarch while Celestia is a gluttonous pig.

In my opinion definitely butt with wide hips. Butts are sexy and when they're big and squishy... Pony can sit on you with this butt, lay on you, you can hug it, there are many options for fun with fat butt. And if we take anthro or human version - wide hips and big butt are much sexier and better looking than huge belly.
And with big butt there are more chances you would be mobile, than with belly brushing the floor.
And bigger chances for getting stuck or accidentally bumping something.

I'm one of those weird people that enjoys fat in its entirely, and likes disproportionate stuff a lot less. I don't like just a big belly as much, and I don't like just a big butt as much. I like it when everything is fatter. I especially appreciate it when a fat pony has a chubby face to complete the picture.

Only thing I could think of to make this story better is if Pinkie viewed Twilight and Twilight's tummy as two separate entities, if that made sense.
