Likely the reason is that most fat people don't care about themselves enough to be decent-looking; after all, they don't care about themselves enough to eat less (though I recognize it's a borderline eating condition and is not as simple as that).

I have in mind, as the opposite, a professor of mine who was fat, pudgy fingers and all, but it was completely obviated by his excellent clothing that suited his body and well-groomed beard that I now realize was probably hide his neck fat. He looked excellent.
