Just looked through the episodes again and it seems the sakurajima started to get phased in at episode 5, it was easy to check, it's the suit featured in the "Hongo Takeshi is Kamen Rider, kaizo ningen" intro. That's way before the accident during filming of 12 (unless, like I said before, they filmed it all as a batch and this batch, out of order).

Unlikely. While he's still in excellent shape for a 70 year old, he's still 70, and they really don't like the lead star doing their own stunts ever since Fujoka himself got into his bike accident back in the first show and they needed to invent a whole new character to replace him.
