It's supposed to be a giant music video for the guy who composed the soundtrack.

You see, Shinya Tsukamoto (the director) is friends with Chu Ishikawa, who has an industrial music band called Der Eisenrost (kind of inspired by Einstürzende Neubauten) and in the late 80s they toured a bit, and composed a few of the songs that ended up making the core of this movie's soundtrack.

So, the music came first, and the movie was based on it.

More generally, it's also a more serious, full-length version of Shinya Tsukamoto's student film (Denchu Kozo no Boken) which was basically a live-action amalgamation of everything that was popular and/or edgy in anime in the 80s (body horror, goofy humor, cyberpunk themes, villains with goth makeup, shounen underdog protag, superhero stuff, guro porn...)

So Shinya just made it more serious and tailored it to the music made by Ishikawa.

Just watch more japanese cyberpunk films like more Tsukamoto (for instance Tetsuo 2) and some Sogo Ishii, some of his best films are Shufle (based on a screenplay by Otomo), Crazy Thunder Road (although those two are barely cyberpunk), Asia Strike Back and Electric Dragon.

Salaryman and his girlfriend hit and run a body mod otaku who gets off on shoving bits of metal into himself. They leave him to die and he curses the MC, who now begins transforming into a literal Iron Man. After killing some girl infected by the same curse his transformation accelerates, he murders his girlfriend with his power drill dick, and he and the otaku (played by the director), alos undergoing the same transformation, get into a death battle that ends with them fusing together into a mobile iron phallus, intent on spreading their gift to the rest of human civilization.
