There were a few games where you are a nanobot fighting inside a body. Microcosm was one, but that one had more to do with the Innerspace movie. But never an RTS for example.
On a similar note, I read about doctors telling children to visualize their antibodies (as spaceships for example) fighting infection as means to help healing.
