If you're talking strictly about piloted mechs, I'd say the AGWS and AMWS units from Xenosaga qualify as being pretty small. Most of the normal models are somewhere around 3m tall, and the cockpit area is pretty snug.

I nominate the Micro Boy from the 2001 Microman line as smallest piloted mech.

The Micromen, in-story, are 8cm tall. The mech transforms into a 1:1 scale Game Boy type portable gaming device, so the piloted mech is about 6 or 7 inches tall.

Indeed. Micronauts, MASK, Rom, Transformers, GI Joe, and Action Man too. So... what, no Visionaries? No Inhumanoids? Eh, they'll probably show up sooner or later.

Unless we're going to get down into the microscopic scale, the 7-inch Micro Boy probably wins for "tiniest piloted mech."

If we ARE going down into the microscopic scale, then the Phobos units (pic related, from Marvel's Micronauts comics) are probably the tiniest. They're mecha piloted by people who live on a planet the size of a MOLECULE. The mecha themselves are probably subatomic in scale.
