I don't have the Histoire DLC yet and though I did get the Nepgear plan (and complete it and meet her and add her to my party, I did use her in combat prior to that), she wasn't present in the battle. Moms in fiction just kind of default into "dead at childbirth" if they're not present and their absence isn't relevant to the plot. It wouldn't be too much of a stretch to explain away Carol's and Tsubasa's moms with that. Explains why the latter has zero memories whatsoever of a mom, as far as we saw. She could have been one of Isaac's homunculi all along, but that's apparently not the case.
Now I feel sad that by applying an overused cliché on the spot I just cared more about Carol's backstory than the writers ever did.
