If there's a good used book shop in your neighborhood the owner probably knows more than a few binders. Also I believe there's a Dover diy bookbinding book!

1: slice the spine off with a razor blade.
2: remove front and back covers.
3: slice through the stitching holding the pages together
4: pull each page out and scan both sides
5: run each page through OCR and dump the results into a text file
6: CORRECT THE OCR ERRORS. most people miss this. ideally the text should flow as it did in the book.
7: convert to .epub using Calibre
8: burn the remains and use that to cook a tin of beans. eat beans, read epub.

Search for book binders in the general area. (and then some, it's a dying trade.) If there's a university nearby, there'll be a binders where students get their theses bound. Even if they don't do fine bindings, they should at least be able to fix up or replace the covers with a decent buckram one which should last a while and look not too shabby. There may also be some amateurs around, see if there are hobby groups or craft fair around by checking online or with local bookshops (not chainstores, independent places), they ought to know.
Failing that, try online but it may be more hassle than it's worth. see about replacing the book, try bookfinder, ebay and a thorough google search, that ought to cover your bases.
