It will make the skin softer and thinner, which usually means lighter complexion as well. It will cause some subcutaneous fat to distribute over the face as well as the rest of the body, giving the feminine squishyness. All of these are associated more with feminine appearances. But it's not going to change the skeleton which is where the most striking and obvious sexual dimorphism usually is. And estrogen generally does a worse job of fat distribution on biological males than it does on natal females. Many in the transgender community are caught up in wishful thinking and are trying to oversimplify or obfuscate what is really involved, or what will really happen. Transition is not so much about becoming physically female (medically impossible at this time) but if the goal is looking "more feminine" (than you started as); then in that context, HRT does something. It does "something".

HRT doesn't really do anything to the bones, but it does soften the skin, cause fat to distribute as it would in women (including the cheeks) and a few other changes. It seems like this girl in the OP didn't really gain any weight at all from start to now, hence why there's not really an effect. Ideally, if you're thinking of going HRT, you should first get as skinny as possible and thus get rid of all of the fat that accumulated in more masculine areas (basically your midsection) and then after a few months, start gaining weight so it can all go to your cheeks, hips, boobs, and butt.

Also, there still seems to be an effect on the face, in that her skin appears far softer and paler in contrast to her lips and eyes, and the eyes got rounder, which goes quite a good ways towards making someone appear more feminine. If only she could wash her damn hair, though; stuff's oily as fuck.
