Since you said they cover one of the surgeries you're half ok, try to save for the other somehow (I also know what I'm saying doesn't help since it's not like you probably have a ton of money laying around).

How are the trans support groups and non nongovernmental organizations around there? Maybe they can help you somehow, is it too hard to get a loan and pay it slowly there?

Also have you thought about trying to do some cam stuff while not showing your face? there's a shitload of guys that pay for for pics and stuff.
