I'm just a biologist, I don't really work on that scale much. I'd hypothesize that, since the brain and body develop at different rates, and very subtle changes in the endocrine system can be amplified tremendously and triggered by all sorts of arcane stimuli, the mismatches that Zheanna and anons are mentioning may stem from that. I guess we'd need to pin down a definition of mental illness. It's hard to put it in the same camp as a lot of other mental illnesses, because while it has largely mental effects, it's effectively treated with physical changes. An important part of that is also HRT, which would make classification as an endocrine disorder arguable. It's not similar to, say, schizophrenia or depression.

If it's useful to you to classify it as a mental illness, go for it, but because it doesn't fit cleanly into that box, a lot of trans people don't want the label, which is arguably incorrect and comes with a lot of extra stigma.

Trannys are the same way. We have a physiological illness that when untreated manifests as mental illness: gender dysphoria. Us wanting to be women or men and change or sex is not the mental illness component to transexuality. The mental illness component is the resulting pain of our miss matched body vs brain.

And actually calling something a mental illness actually contributes nothing to a doctors diagnoses, patients information, etiology, efficacy of treatment, etc. it's a pretty useless word that has a tremendous amount of stigma.

Almost always when people ask if being transgender is a mental illness, they mean the desire to change sex. The desire to change sex is not the mental illness component, that is the result of our physiology. The dysphoria when left untreated can be labeled mental illness.

It's a chicken vs egg thing. We feel repulsed by our bodies due to neurology and physiology, and that repulsion when untreated and forced to love with that then manifests into mental pain.
