Most people have a very basic understanding of the characteristics of each gender. For them a weird skull means just that, a weird shaped head. Maybe even weird = ugly. But not necessarily weird = guy with long hair.

The same with voices. Deep = Male, high pitched = Female. Of course someone with knowledge on the subject may be able to tell exactly why those weird things are happening, but most people aren't able to tell that.

There's nothing biological about human's perception of gender. The phrase "social construction" is fucking cancer and it's been bashed to death. But the perception of gender is definitely of the few things I would realistically consider a "social construct".

If literally every human female started to shave their heads and wear baggy sports pants and tennis shoes, then, over time, the perception of what a female is would adjust to that. Just like the perception of females had to adjust from what it was 200 years ago to what it is now.
