The difference is like Disco Stu on the Simpsons vs Marge on the Simpsons. One is a minor character who's only brought in to be laughed at, basically just wacky set dressing. The other is a character who can be the butt of jokes, but also has storylines and sometimes even serious moments. One's comic relief, one's a sympathetic character.
Most of the gay people on TV back then were just the amusingly queeny gay who didn't have much of a role beyond being amusingly queeny. Contrast this with Will and Grace, which started airing the year after Ellen came out and the producers credit Ellen's coming out as helping them get on the air, which is a sitcom that features queeny gays being amusingly queeny, but they also have plotlines, character development and sympathetic moments.
