if someone truly believes they are a woman or a man to the extent of taking hormones/having surgery then why would they not socially identify as a woman or a man?

no, this does not mean a trans person who perfectly passes should not disclose the fact that they are trans to potential romantic partners. it just means they shouldn't be advertising the fact that they're trans either.
