I keep hearing about new advances in the medical field.
Stuff like stem cells, cloning organs, genetics, etc.
How much of this do you think could be applied to trans people (both FtM and MtF) ?
Could we one day see better HRT ? Better SRS ? When do you think this could start being done regularly ?

I think it might be a shitty fact that as soon as science is at a point when things can be made very comfortable for trans people will be the point the science is advanced enough to prevent the question in the first place.

Genetic engineering is a when and not a if at this point.

I'm sure new technologies will arrive, but I wouldn't try to plan your transition around it. I.E., don't wait on SRS if you want it for some possible future new technique that may or may not arrive in time to help you.

Personally, the trans medical advancement I want to see is a way of detecting it earlier in the brain. What you do with that information could be debated. Do you interfere with the brain to change the gender identity to be cis? Do you use that information to make sure all trans people transition early?

If transsexuality has a biological basis and isn't just a defect in development/socialization, then medical science will eventually find a way to cure it or prevent it from happening by modifying the embryo.

trans surgery can be expected to progress over the next 40 years similar to how it has the last 40, with steady improvement punctuated by occasional breakthroughs
mtf and ftm surgery will continue to improve at the exact rate they have, with mtf being only distinguishable by a gynecologist if done well and ftm not being at that level but being far better than generally perceived, and the other surgeries in that constellation (e.g. ffs) will improve at about the same rate
i suspect the next major breakthrough will be facial masculinization surgery, which in the current social context is primarily considered an issue of vanity but its necessity will be seen when the death of ftm passability that is currently beginning similar to the 80s death of mtf passability reaches a critical mass

I think it's not quite on that level yet. It can be helpful if your genital dysphoria is so strong you absolutely disgusted with your penis and can't even enjoy sexual arousal, but otherwise it's just not worth it at this point in medical development.

Lubrication can happen depending on the surgion and the patient, but it's not a guarantee and it's rarely enough on its own. There are dry cis women too though.

-skin reconditioning
-better depiliation
-organic breast implants made with actual tissue that grows more more smoothly or at the very least other breast implants that feel real and/or leave you free of the slightest worry of them breaking
-bone fixing, likely bone shrinking, or at least epiphiseal re-opening for select bones, or more generally for transmen. And aside from long-bone growth, making the hips resume growing of course.
-or better bone grafting at least, again useful for hips.
-better AAs... presumably actually right around the corner with generic Abiraterone!!
-better hormone treatment, simply adjusting the gonads to produce the opposite steroid
-fat redistribution supercharging
-better voice treatment
-better hair repair

Of course, this kind of shit will immensely turn the lives of all people on their heads, make everything so chaotic, meaning-robbed, overwhelming, authority-empowering and impossible to keep track of that humanity will just turn on it and burn it to ashes. It'll make being trans so convenient and interesting, normal people will be jumping into it more and more, or rather, rich people, or they'll get someone close to sponsor, likely turning them into show-dogs.

Buccal or stem cell vagina SRS maybe within 20 years, maybe.

Stem cell breast implants perfected in 10 years, these already exist but are not widely used for some reason.

Sperm and egg cells from skin cells within 10-15 years, allowing trans people to create genetically related children, doesn't solve the problem of needing a womb though.

Womb transplants for mtf and penis transplants for ftm, possibly. These are being tested on patients now but there's not a solution yet for organ rejection. Maybe within 10 to 20 years that will be solved.

There will probably not be many advancements to FFS, or anything involving skeletal shape.

Theoretically research into all these areas should start to speed up as there's a fairly sizable growth in the trans population now, meaning a larger market share making research worth the $. Then again there may be a huge swell of disillusioned detransitioners in the next 10-20 years too. Or a war, or a major cultural shift.

Virtual reality is probably a better bet.

It all hinges on how discerning the public is though. People won't spot a trans vag if they don't expect a trans vag to exist or look good, they might just think it's a funny looking cis vag.
Just like how ye olde trannies pretty much just had to wear opposite sex clothing to pass, because someone doing such a thing was just so unexpected that the puclic just though "that person in a dress is one funny lookin' dame." These days, cross dressing is much better recognised, so people are able to actually ask themselves "wait, is that person male?" and spot evidence of it.

It's like how languages with different words for similar colours actually result in people being able to better see the difference between those colours. Or how cgi that seemed incredibly realistic a few years ago quickly becomes or dated.

I think it sort of goes both ways though. With higher trans visibility comes better ability to clock someone as trans, but at the same time, with higher trans acceptance comes better ability to see what gender someone's presenting as and not question it. The people that I saw as "men in dresses" when I was a kid and didn't understand what trans was, I'm now able to instinctively see as women. Trans women, sure, but whatevs.

Oh god, when writing that I completely overlooked the fact that your old body would be walking around with someone else's brain in it. I think that'd creep me the fuck out. Might be worth it, but I think we'd have to live on opposite ends of the earth or something. But someone happy with that body would probably make it their own. Brighten it up with those post-transition eyes, make the most of its natural features, carry it in a new way. Maybe it wouldn't be too weird after all, especially with my own new body to make my own, making any hang ups about the old one kind of obsolete.

There is no Myself I am "supposed" to look like. The body that would have existed if I were born female does not exist; the body that would have existed if I got on hormones before puberty does not exist; the body that would have existed if I got on hormones before puberty does not exist; the body that would have existed if I got on hormones 10 years ago, 5 years ago, 3 years ago does not exist. I look like myself tautologically, and looking like myself causes me so much pain. Why would I have any attachment to that?

I think HRT is probably as good as it can get. SRS has a lot to improve on, and there are a lot of advancements in the past few years around surgeries that have yet to be applied to it.

Bone reduction surgery might be a thing in the future, but honestly its at a very primitive level right now. However, FFS will continue to get better results, as improvements are made to plastic surgery in general.

More novel things that might happen soon might be : neo-ovarian transplants (basically scientists crafted pseudo-ovaries that are capable of producing female hormones endogenously although not eggs), gene therapy which could change existing gonads (something similar was done in rats), and more advanced gene therapy which could in theory remove or insert a y chromosome (although I don't know of any example where this has happened, chromosome editing isn't technically impossible)
