Lose a bit of weight, get your hair cut into a slightly more definite 'style', clear your skin up a bit (Use BB cream in the interim because it won't happen overnight).

Other than that you're fine really. Realistically speaking weight, skincare and hairstyle are pretty much all you can do unless you're trying to get a more specialized look (In which case, you'd know what you're going for).

Be happy - You're not ugly, you've got all the foundations and you're not trying to work around some kind of glaringly bad feature or permanent issue. You just look like you've neglected your appearance for a few years; the more work you put in the more you'll get out.

I won't tell you how to style your hair or what to wear because that's a personal thing unique to you; go with what you like the look of and gives you confidence. Beware of people that give you hair/clothing recommendations - they can tell you what will make you look good in a picture, but you'll come across very awkward and underconfident in real life if it doesn't suit your personality.
