I've stopped my own pulse with one on four different occasions and a buddy's pulse once in training, so I know they work alright. Plus if just one doesn't work on an especially fat/muscular person, I have another. I keep them both wrapped around my leg under my pants. Also RATS is great for using on kids and animals. Nothing beats them for EDC imo. If you carry a CAT all day any day then more power to you, that's just not me with my work and my wardrobe requirements

You're extremely unlikely to lose a limb if you get medical care within a few hours.
It's possible for the veins to retract below the wound and cause the tourniquet to need to be placed higher than you would think. Telling untrained people to put the tourniquet high is a safe suggestion.

And doctors have an occupation in a certain area of medicine. They're not all field/trauma medicine specialists.
The realization that tourniquets really aren't that bad to use for a few hours is relatively recent and took place with military studies (as with a lot of trauma medicine). Older doctors or simply doctors in an unrelated field may not know the details of modern field care.
If a doctor is treating you they can use a fancy controlled pressure cuff for a tourniquet and apply it wherever they want. In the field you're not going to be able to properly evaluate where to put the tourniquet and putting it higher than necessary is much better than lower than necessary. That doesn't mean you need to put it all the way up your limb for any wound but giving it a good 5 inches is a good idea

Most guidelines (written collaboratively with physicians and other professionals) I've seen lean towards high and tight. Mainly because it's a) easier for laypeople, b) allows quicker administration as it might take time to identify wounds and c) because arteries are elastic and can contract upwards away from the wound, giving you an inaccurate mark for the TQ site.

Also, it's generally accepted these days that you can have a TQ applied for around 2 hours minimum without an lasting neurological damage. The longest case I'm aware of was for, I think, 6 or more hours with minimal damage.

Haemostatics aren't really a replacement. By "better shit" he probably meant some sort of controlled pressure cuff they might have in an ambulance.

Agreed, neither of those are "replacements" for a tourniquet.

I think the EMS improvised TQs varied, but most were rubbing tubing with forceps acting as a windlass. A BP cuff definitely would have been viable (a service near me specifically encourages the use of one as an arterial TQ). The biggest issue I see with it though is if it's pretty old, the velcro might give way.
