Like always any conflict would quickly turn into a proxy conflict, so basically WW3 between the US and Russia.
In a weird completely theoretical situation where solely Russia and solely Japan went head to head, I imagine the Russian Air Force would triumph, fuck up their navy, and their ground superiority would let them invade conventionally and the JGSDF wouldn't be able to stop them, nor would the Japanese likely have enough morale to want to.
Most of Japan would probably be apathetic to Chinese landing in Tokyo and replacing Abe with slightly yellower Abe as long as they still have beer and anime. Give them a few more decades of militarization and postmodern apathy.

there is a big difference between "Japan wants to have control over vladivostok and extract concessions" vs "There was limited escalation after an incident in the kurill islands and neither nation wanted to back down but neither was willing to expand it past naval conflict".
