Looks like a generic Cold War era (civilian?) mask.
A black Russian GP-5 mask would do a passable cosplay.

Swapping models ain't even difficult, but CoF did an amazing engine overhaul to the GoldSource engine in general. Normalmaps, water shaders, dynamic per-pixel flashlight... shit could pass as an early Source mod at best.
