What causes a failure to bring a slide back to forward position with round NOT hung up on the barrel entrance. stops like 2-3mm before being set to fire. a light tap on the back of the slide pushes it forward for the most part.

when is this failure happening? intermittently during fire, as you rack the slide, etc.
Buy a new mainspring and see if that fixes your problem. It should.

Sounds like your recoil spring is worn out or maybe the rails that the slide is mounted on are dirty and gunked up or need to be polished. What gun? Remove the slide and see if the rails are dirty and order a new spring.

start by isolating certain components. change out the magazine and ammunition, if you can change the shooter - maybe you're just a faggot.

you mean feed ramp. it could be a number of things, worn out spring, dirty/ragged slide rails, bulged barrel, would be my first checks.

its happen irregularly through 8 round mags we own on a taurus pt 1911.
its not a new gun and the slide seems pretty well worn in but it still coudl be that.
