It's not necessarily about killing the most people, it's about causing mayhem and scaring the shit out if everyone that's not directly effected. Will a truck bomb cause a tunnel to collapse completely? No, probably not; but it will cause significant damage and still kill a couple dozen rush hour commuters. Shutting down a major tunnel for a week or two will have disastrous effects for the local economy, and it will cause law enforcement to permanently allocate more funding to preventing another such attack at a critical spot like a tunnel.

The question then becomes where you control it from, and how do you connect to it wirelessly in a way which isn't easily jammed or destroyed.
