It depends what grenade, the Mills bomb that killed Jason Dunham is an old style large "defensive" grenade, which generally has more explosive and bigger fragments than a modern "offensive" grenade. It weighs 0.76 kg, compared to its contemporary American 0.6 kg Mk2 grenade, or the modern American 0.4 kg M67. It's possible that the fragments of a smaller offensive grenade might have been successfully deflected by a helmet.
