This is a Russian MiG-29 experiencing an engine failure due to a bird strike. The pilot survived thanks to Russia's really good ejection seats. Nobody died. The pilot stayed in the aircraft that long to make sure he pointed it away from the crowd.

Depends on the make.
You can set command ejection in some aircraft so that any initiated sequence will cause both to eject. Usually on training aircraft (PC-9/21/T-6/F-18B) the instructor will turn it off due to the chance that a student will accidentally begin the sequence.
