Depends. On my AR they aren't. On my 12ga they definitely are.
its up the the individual. sometimes the fore end can get too hot to the touch. sometimes the fore end can be too slippery. sometimes, as with some quad rails, the fore end can be too sharp or uncomfortable to handle.
adding girth to the fore end is also a possible consideration.

Not really but they aren't comfortable to hold.
When I had a quad rail I put covers on the rails to make the handguard fatter. The covers gave me a good, comfortable place to hold. They also protected the rails from having bumps and burs if they were hit or bumped into by something.

Entirely. Without them the gun gets mad at your poor fashion sense and refuses to fire.
Not really. Well-designed quad rails have smooth edges or scallops on the Picatinny sections to make them more comfortable (see DD RIS II). The problem is this increases machining costs, which transfers to the end-user. Getting an M-LOK or Keymod rail eliminates this issue while also cutting down on weight.

No, at least not on picatinny. They make the gun too fat to get a real tight grip on if you're using an AFG or handstop. Just get a ladder cover if you are really worried about it.
