Sole cushioning, sole construction, padding, type of leather. Modern boots also have membranes sometimes.
I really much prefer old school boots for cleaning, durability and looks, but I'm not sacrificing my knees for that. The irony is that it's perfectly possible to make a welted sole with cushioning layer, just not very common.

Vietnam jungle boots look cool sure, but they're a nearly half century old design made with the intention of being mass produced and thus only "alright" instead of really good.
Good, modern hiking boots will hurt your wallet, but they'll save your feet and knees, and if you fuck those up it's gonna cost a lot more than $300 to replace them.
Buy the most modern, highest quality and best boots you can. Never be tempted into military surplus because it really isn't a good quality.
For Borneo I bought US jungle boots, which were much better than what we were issued, from a US serviceman on R+R in Hong Kong. They were about as good as they got back then, but since that point technology has come on massively. The idea that someone in the 21st century would wear issued equipment from 50 years ago is idiotic. Most military equipment is of a poor standard and boots are of no exception, it really isn't good and I say this as someone with years of practical use.
I have worn Meindl boots for decades and I have been consistently impressed. Go with them (or other, similar brands) if you value your health, time and employment.
