Sometimes it's pure bad luck, or the result of commanders taking a calculated risk that was reasonable under the circumstances.

Either way, the US drops the most bombs, so it hit misses the most bombs.

We'd need more details of the specific incident to make an informed judgement.

Also, this goes without saying, but Canadians deserve to be killed on general principle.
