This. Watch any videos about Israeli carry or any tactical handgun training videos and they will show you how to use the rear sights to rack the slide by hooking it on the heel of your boot or something on your duty belt, etc.

There are ways to rack a pistol without using both hands, and there are ways to rack a pistol with no hands, although some methods are more difficult than others.

As far as keeping one in the chamber, I carry in condition 1, but when I'm at home it's unchambered because there's no reason to have a round chambered in my home that's already fortified with burglar bars, deadbolts and barrel bolts. Nobody's getting to me before I can rack the slide back in the half-second that it takes to do so.
