Because I don't live on Okinawa I'm not really worried. If it was still the cold war I'd plan on driving to ground zero of the nearest likely target (probably Boston or Pease AFB for me) set up a lawn chair and wait for the flash. I've got too many health problems to survive in a post-apocalyptic world.

Well, I live in a rural town that is an hours driving distance from a large city on the east coast.
Would I be instantly fucked or do I have a chance at waiting out the fallout? And would I need a bunker at that distance or just a basement?

Can you imagine you show up at one of those fallout shelters and there is no food or anything because none of them have been stocked for years.
The onbly Statewhich is in danger is Hawaii, because there isn't anything worth hitting in Alaska. The half dozen of nork nukes wouldn't cause considerable damage anyway.
