I recall hearing that Miculek ran some training for SF, not sure if it is true thought. In any case, I'd prefer if team Miculek/Hickok came for me. They would at least put me down quick and clean.

Jerry Barnhart makes a living off DOD training (notably a lot of NSW guys over the years)
Rob Leatham has been known to train Delta guys in pistol
John Shaw of Midsouth has been doing exclusively closed-enrollment classes, primarily SOF guys, for decades now
Houston Shaw does a fair few DOD classes as well

Leatham is the most famous one of the bunch but all of them (except Houston Shaw) were IPSC/Steel Challenge badasses back in the pre-USPSA days. In Houston's case he's John Shaw's kid and a Steel Challenge badass in his own right.
