People wise, all that usually happened was falls. We had a maintenance platform tower that was part of an old lumber deck that could be moved around with a fork truck. Worked great because you could set it next to or under areas and the guys had a solid working area with places to walk around and work. Anyway one retard wanted to play Titanic and ride around on it with his arms stretched out. The fork truck hit a bump and the platform tipped, and he went flying. The truck stopped, the platform slid, and he got mangled from the fall.

And again, new round of safety BS and piss tests.

Guys would get hit by boards that would get caught in chains and swing around.

Saw our industrial chipper explode once. The wires for the metal detector before the chipper were frayed so the detector wasn't working, and a sledgehammer head went into it. The metal detector shut off the shaker that moved shit down. Anyway, the chipper was a 3 ton steel drum that had 8 or 10 blades and spun at 12k rpm once fully spun up. Chucks of that fucker went flying everywhere. the drum went off balance and broke free. I was on the deck when it happened and saw everything. Scary but cool.
