The theroy is that somehow, say a defect in the magwell or one of the bullets being longer than spec, the top cartridge got stuck and left behind when the mag came out, and when he racked the slide to clear the rifle that stuck cardridge got chamberd. Now that I'm older I think that maybe it was just a story he told me to get me to be thorough when I clear weapons. If that's the case, then that's 0 nd's in 40+ years. I'm in my late 20's, and no ND's even though I carry. ND's are preventable with good practices and attentive behavior. Don't handle guns "on autopilot", pay attention to what you're doing, especially when holstering or clearing.
