Knights have protective gloves that will give them some protection. Most unarmored opponents wouldn't fight that way.

There's a guy on Youtube that talks about it, I think maybe Skalligram, but basically you can do it with a reasonably sharp sword. Swords generally weren't razor sharp, so it's not going to just magically slice you up on contact.

Basically all blades need to be drawn or pushed to slice, so if you have a firm grip the blade isn't going to cut you open.

Further, that style of fighting would be almost exclusively used against an armored opponent, since their armor makes almost all slashing attacks useless and lots of stabbing attacks uneffective. So you need to do impact damage or stab into the gaps.
