Standard uniform is flags on the left arm only. Flags are made to appear as if being flown while advancing. Hence, issued flags are "backwards". The reflective vest is for perimeter guys. In an emergency in an urban center, the last thing you want is unidentified guys with guns complicating things. They might be wearing armor under that, or soft armor under their shirts. Using what they got.

Balaclavas for protecting their identities. A big terrorism event has a lot of cameras on it. They don't want to be ID'd publically like that.

It's neither aesthetic or "operator", but real life isn't always aesthetic and people aren't absolutely perfectly equipped.

I wasn't thinking anything that technical. if you look at the picture, the major giveaway is the regular shape of their heads and the regular eyeholes. Ironically, the black comms straps on their headsets actually helps to break up the shape of their heads, but the black oval eyeholes in the pure white balaclavas stand out. Nature doesn't do pure geometry very often.

So they just need to apply the same outline disruption as the rest of their camo and have printed balaclavas with irregular black patterns.

I'm not so sure. You can take it to ridiculous extremes. The shape of the hands holding weapons are just irregular-shaped blotches of white, whereas the heads were regular and rounded with stark oval eye-holes.

You could also argue for the chest packs as well.
