Basically they work with guide rods that hold the shot cup closed until it clears the suppressor. Can also work with slugs, however buck without a shot cup will blow it up.

Price, affect and weight are kinda lopsided, and I wouldn't recommend one for a practical purpose as you're effectively spending a nice rifle price tag to put extra weight AND length on the end of your barrel just to reduce the sound by a incy wincy little bit.
