It can take years before people even start to feel any symptoms, but by that time you might have only a few months left depending on the type of cancer. That's why if you are in a risk group for cancer doctors recommend that you have regular screenings.

So yes, people have been smoking for millennia, but during most of that time medicine has been pretty shitty so people simply didn't live long enough to die of cancer if that makes any sense.

I don't want to sound like some conspiracy nut, but if you google nuclear testing and tobacco, there are some pretty interesting reads about how radioactive alpha particles wind up in fertilizer and gets absorbed by the tobacco plant, obviously if large amounts of it gets in your lungs it could cause problems. Theres also claims about a global increase in cancer after nuclear testing and tobacco use was possibly used as a scapegoat seeing as it was such a common habit years ago.

There is obviously an increased risk of certain cancers with tobacco use, although it mostly comes from people who are already at a predisposition to get it and heavy users. Heart and arterial diseases are much more common in heavy users than cancer.
