Cut down makarov grip
re-worked ked rreciever. Possibly a teliscoping bolt because there is no real room for a chamber in there and the bolt would need the extra mass given that its a blow back.
But the bolt would need to be like tungsten if it was blow back given the recoil energy of the round it uses. It could use a flapper lock or something as an action

It's quieter than an unsurpressed one at least.

In Metro the enemy can hear silenced gunshots just fine from a fair distance, so the justification is 'it's still kind of loud'.

The game doesn't show it but maybe it has the action of a nagant revolver, where pulling the trigger actually cams the cylinder forward to close the seal between chamber and barrel.
