Slide A looks like a Kel Tec, SCCY, what have you.

Slide B looks like a Glock brand Glock.

Both work fine.

In this drawing slide A narrows down to a comically small diameter, though.

That's there for the purpose of preventing barrel from contacting the slide when tilting so it had room to move, like >>32370739 said. The first gun to have a flared barrel like you're mentioning is the Detonics combat master.

Compact guns like glocks that have a straight walled barrel have the hole in the front of the slide somewhat ovular, with extra room at the top for the barrel to tilt into. As far as why somebody might make a flared barrel is for 1911 style guns to not have barrel bushings, and arguably better accuracy with the slide having contact all the way around the barrel.
