There are probably improvements to the refueling methods to reduce accident rate. If you make a task very difficult and easy to fuck catastrophically, you're going to have lots of "pilot error."

V-22 has fuckhuge rotors, anything that goes near a V-22 has an enormous chance of hitting a rotor.

This. Marines are like rabid dogs. You release them into any shithole in this world and let them shoot, blow, rape and pillage everything in their path. Then you put them back in their cages a.k.a barracks, give them some weighs to lift, drugs and money to keep them occupied and civil.
