There are probably improvements to the refueling methods to reduce accident rate. If you make a task very difficult and easy to fuck catastrophically, you're going to have lots of "pilot error."

At some point when you have so many crashes due to pilot error (nearly all of them so far) you have to ask if it really is an inherent flaw in the aircraft; is it really that difficult to fly that it becomes a danger?
