That's a mossberg 500, you can tell by looking at the location of the safety, and the flat bolt. As for the wood, I assume you could have a gunsmith, or maybe a carpenter help you out there. It looks like they took a mossberg 500 with a vent rib barrel and wood furniture, cut down the barrel, and checkered the wood.
