Check out Michael Mann's filmography, specifically Heat.

No ear protection, Val Kilmer really fucking let loose with a CAR15 from inside an automobile, hurt his ears, hell, they all got some permanent hearing damage from making that movie, Al Pachino shot his gun in tight spaces too.

The sounds are all 100% genuine too, they didn't dub the guns over with stock sound effects (Mann even reprimanded an editor who tried to do so).

Speaking of Val and the CAR15, he does expert reloads of it.
