It's tangential, but I recall reading something along these lines:
British Prime Minister Callaghan (Sort of on the left of the Labour party, fought against tougher trade union regulations in the 60s, then ironically got brought down by union strikes they would've prevented in the 70s) would've ordered a retaliatory strike in the event he'd had the information that a strike had occurred or was imminent on the UK.
Dennis Healey, his chancellor (On the unambiguous right of the Labour party and once in charge of defence, though he was responsible for implementing spending cuts implemented from above.) wouldn't have ordered a retaliation in those circumstances as the deterrent would've failed in the role of deterring had that happened. (He only revealed this after he left office.)
Regardless of whether or not it's accurate, I've always liked the "Healey strategy" for nuclear deterrence: Insist entirely that you will fire if fired upon, give absolutely no indication this wouldn't be the case. If you're fired on anyway and defeat is imminent, well, no point firing back. (Though in the UK we run into the problem of letters of last resort, as actually writing "do not fire back" in the letter does technically induce the risk of giving away your position too early, should someone somehow obtain access. For maximum deterrent value, you'd have to bluff it right up to the point of standing in front of a physical button yourself.)

No. The reasons are you don't know your missile's target. I could be signing off a bunch of non combatants to death in a major city with no military value. If I end up letting a bunch of military targets survive such as a base or concentration of soldiers they are a resource that can assist post exchange and render aid to affected areas. After the exchange if there is a standing government left on either side will call a cease fire anyway. I know there will be enough other people that will turn the key and kill millions of people and it's something I don't want to be part of.
