The trunion cracked on my PTR, causing the bolt-gap to shrink to zero rapidly (Within the span of a magazine). The lack of a bolt gap caused fuckery with the extraction that allowed chamber pressure to press the casing into the flutes.
There were some other cycling issues involved, like this reverse double feed.

Had the weirdest double-feed happen to me the other day. The round got ripped in half by the extractor and caused pic related. It almost looks like the manufacturer attempted to piece together two previously-broken halves of brass and proceed to make it a live round. Definitely one of the weirdest things to happen with my AR.

thats a broken shell on extraction not a double feed...clean your chamber it could be overly dirty. if that keeps happening, you may have an issue with too much gas or a rough/ chamber. usually see that with steel case because of the coatings they put on the shells
