I was really looking into buying a pistol ans was wondering if you guys had any input.
I've been looking around, mainly at SIGs and other modern firearms, but something turned my attention towards Makarovs.
Are they particularly reliable? Should I stay away from a certain country's Makarovs? Is there anything I should really be worried about when buying one? (I'm aware of the proprietary, possibly expensive round and heel release, I'm ok with those.)

They're reliable as fuck and an excellent in between between a pocket pistol in .380 and a compact in 9mm.
They're nice and thin and if you use pants that aren't too tight you can pocket carry them.
The trade off is that it only carries 8 rounds though...

Only slightly higher than the price of match grade .22. That said, expect to pay more than a dollar a round for hd ammo.

I know it isn't expensive at the moment, it's just that it isn't very widely used in any modern firearm.
I'm just a little worried that it might cease production soon, though I'm sure that I could probably buy plenty before that point.

All the cheap plinking ammo is made in the Eastern bloc, and will probably continue to be made for the next 40 years... I have trouble finding it locally, but you can always order from AIM Surplus or other sites.

The only reason to buy Soviet or Chinese is for added collector's value or novelty.
Bulgarians are the cheapest and some people think East German ones are better made but for the most part all Maks are created equal.
